It is easiest to download the Word application and fill it in, then either print it out or email it to BGoodhues@towson.edu along with supporting documentation. A completed application consists of:

• Completed application forms: if you are a student, a faculty member must review and sign your application form. You may email the application to BGoodhues@towson.edu, and send a hard copy with signatures through the mail or via fax to (410) 704-4494.
• An Informed Consent Form or informational disclosure cover letter, as appropriate
• Supplementary material including copies of any materials to be given to participants, tests, surveys, or questionnaires, recruiting flyers, interview questions, and debriefing material as appropriate.
• Permission letters from the authorizing official for any research taking place venue where permission of an authority is necessary. (Example: if you are doing research in a school, permission from the Principal is required.)
