Recently, several applications have been delayed due to problems caused by applications obtained through third parties. In order to prevent confusion and keep the review process running smoothly, we ask that you simply link to this page, rather than hosting your own copy of the application. Thank you.

It is easiest to download the Word application and fill it in, then either print it out or email it to ours@towson.edu along with supporting documentation. A completed application consists of:

• Completed application forms: if you are a student, a faculty member must review and sign your application form. You may email the application to ours@towson.edu, and send a hard copy with signatures through the mail or via fax to (410) 704-4494.
• An Informed Consent Form or informational disclosure cover letter, as appropriate
• Supplementary material including copies of any materials to be given to participants, tests, surveys, or questionnaires, recruiting flyers, interview questions, and debriefing material as appropriate.
• Permission letters from the authorizing official for any research taking place venue where permission of an authority is necessary. (Example: if you are doing research in a school, permission from the Principal is required.)

Applications may be turned into the IRB Administrative Office, Enrollment Services, Suite 208, emailed to ours@towson.edu with signed copies to follow, or faxed to the attention of the IRB at (410) 704-4494. Questions may be addressed via email to ours@towson.edu, or call (410) 704-2236.
