How to obtain an IRB application:

Attention! Towson University Faculty, Staff, and Students: Please do not host versions of the Towson IRB Application on your own website.

Recently, several applications have been delayed due to problems caused by applications obtained through third parties. In order to prevent confusion and keep the review process running smoothly, we ask that you simply link to this page, rather than hosting your own copy of the application. Thank you.

NEW POLICY: Student Principal Investigators must submit a completed Faculty Research Sponsor Agreement form with their IRB application. You must submit documentation of completion of Human Subjects Research training for all study team members with your IRB application (http://grad.towson.edu/ours/irb/irbTraining.htm).


CITI training must be completed EVERY THREE YEARS in order to continue conducting human subjects research at Towson University.

It is easiest to download the Word application and fill it in, then either print it out or email it to irb@towson.edu along with supporting documentation. A completed application consists of:

  • Completed application forms: if you are a student, a faculty member must review and sign your application form. You may email a signed pdf copy of the application to irb@towson.edu, or send a hard copy with signatures through the mail.
  • An Informed Consent Form or informational disclosure cover letter, as appropriate.
  • Supplementary material including copies of any materials to be given to participants, tests, surveys, or questionnaires, recruiting flyers, interview questions, and debriefing material as appropriate.
  • Permission letters from the authorizing official for any research taking place in a venue where permission of an authority is necessary. (Example: if you are doing research in a school, permission from the Principal is required.)
  • Please contact the IRB Administrator/OSPR prior to submission of applications approved by outside Institutional Review Boards.

Applications may be turned into the IRB Administrative Office, 212E Administration Building or emailed to irb@towson.edu. Student IRB applications must:  include a signed Faculty Research Sponsor Agreement form.  For electronic IRB submissions, students must cc the faculty sponsor in the email correspondence.
