It is easiest to download the Word application and fill it in, then either print it out or email it to email@example.com along with supporting documentation. A completed application consists of:
Applications may be turned into the IRB Administrative Office, Enrollment Services, Suite 208, emailed to firstname.lastname@example.org, or faxed to the attention of the IRB at (410) 704-4494. Student IRB applications must: include a signed Faculty Research Sponsor Agreement form. For electronic IRB submissions, students must cc the faculty sponsor in the email correspondence.
