Doctoral Forms

Change of Committee Policy and Form
Used when changes need to be made to an established Committee. Contains the policy regarding this action and the form needed to complete it.

Committee Appointment Form and Policy
Outlines the requirements for a student's Doctoral Committee and documents the selected committee. Contains the Doctoral Committee policy and form.

Comprehensive Exam Announcement Form
Used to announce a student's ability to begin comprehensive exams and to schedule the completion of each exam artifact. Contains the Comprehensive Exam Announcement Form.

Comprehensive Exam Defense Policy and Form
Outlines the procedure for the Comprehensive Examination process. The included form is used when a student has successfully completed all Comprehensive Exam requirements. Contains both the policy and form.

Dissertation Approval Form
Will be sent to the Graduate College (with the Dissertation Format Review sheet) for dissertation approval. A copy (on 25% rag paper) will be included in final dissertation.

Dissertation Checklist
Provided by the Graduate College. Should be completed after a student has successfully defended his/her dissertation.

Dissertation Defense Announcement Form
Taken from the Graduate College Thesis Manual.

Dissertation Format Review Sheet
Contains the dissertation format review to be submitted to the Dean of the College of Graduate of Graduate Studies and Research. It should be submitted with the Dissertation Approval form.

Dissertation Proposal Policy and Form
Contains the policy regarding a dissertation proposal. The included form should be completed after a student's committee has approved the dissertation proposal.

Alternative Dissertation Policy
Describes the specific content and formatting requirements for those students wishing to complete an alternative dissertation rather than the traditional dissertation.

Dissertation Defense Grading Form and Policy
Used to assign a grade to a studentís dissertation defense. The corresponding policy provides information about passing or retaking the oral defense.

Dissertation Grading Form and Policy
Used to assign a grade to the written dissertation specifically. The corresponding policy describes the different passing levels a student may receive.

Doctoral Program Policy Handbook
Instructional Technology Doctoral Program Handbook updated for the 2011-2012 academic year.

Internship Guidelines
Outlines the purpose of and requirements for internships. Includes the Internship Guidelines document.

Prerequisite Advising Sheet
Used to track the student's completion of prerequisite courses. Includes the Prerequisite Advising Sheet.

Program of Study Form
Used to track a student's completion of required courses. Contains the Program of Study form.

Recommended Specialization Courses
Contains a list of courses that are recommended for specific specializations. The list is neither mandatory, nor exhaustive, but is a compilation of suggested courses.

Subject Specialization Policy and Form
Contains the Specialization policy that outlines the requirements for completion of a specialization. The included form is used to track the completion of these requirements. Contains the Specialization form and policy.

Thesis Guidelines
When formally working on your dissertation use this as a guide for submitting dissertation copies, creation of the signature page and necessary Graduate College forms. As soon as the official "dissertation" guidelines are approved, they will be posted here.
